PLEASE NOTE:
1. Disclaimer: The information included in this memorandum is correct on the above date. All information is subject to change by the Pennsylvania Department of Education.
2. This form is required, regardless of whether or not the candidate is securing Pennsylvania certification.
3. Pennsylvania Department of Education & Messiah College regulations stipulate that certification applications and Program Completion Letters cannot be processed until ALL of the designated requirements are successfully completed, submitted to the Teacher Certification Officer and verified. (Verification normally takes 14+ days following the end of the student's final semester and after grade are reviewed by the Registrar's Office.)
The designated items are all required before we can sign-off on an application to any state or employment agency. Failure to complete any of these items could significantly delay the candidate's certification or employment application.

1. SECTION I
Candidate signs and dates this form attesting that he/she has read and been apprised of the required policies, procedures and due dates for Messiah College teacher certification program completion for the current academic year as published on the website of the Messiah College Teacher Education Program -Teacher Certification Information.
2. SECTION II
Academic advisor completes the requested information, dates and signs this section.
3. SECTION III
Academic advisor completes the requested information, if needed. PLEASE NOTE: any incomplete coursework may delay graduation and certification.
4. SECTION IV
The candidate requests, pays for, secures and keeps the original clearances. The candidate is required to submit COPIES of the requested "Criminal History Clearance Forms" to the Teacher Certification Officer. Please indicate on this "Verification Form" when the clearance checks were sent to the required addresses and include a copy of the results.
5. SECTION V
The candidate provides the requested Praxis I & II information.
6. SECTION VI
The candidate's college student teaching supervisor completes and signs this section. College supervisors are asked not to sign this form until sometime during the last two weeks of student teaching. It is recommended that the candidate secure this signature in person rather than via mail or campus mail.
SECTION VII[This signature will be secured by the Education Department secretary.]
This section is completed by the Director of Teacher Education verifying that the candidate has met all requirements stipulated by the Director of Teacher Education Office.
SECTION VIII[This signature will be secured by the Education Department secretary.]
This section is completed by the Teacher Certification Officer verifying that the candidate has met all requirements stipulated by Messiah College and the Pennsylvania Department of Education.
